Why is this so important? After hearing from the community, members of Connecting Communities work together to prioritize the issues to create the 2026-2029 Walworth County Community Health Improvement Plan (CHIP). This plan will include the work of numerous organizations and community members to put the feedback given in the CHA into action, with the end goal being a community where residents and visitors will feel healthier and more connected.
